Minimal Change Disease (related)

Minimal change disease is considered autoimmune-related or immune-mediated. It affects the podocytes of the glomerular filter, and current understanding proposes a T-cell-driven process in which podocytes begin expressing a protein called CD80 (also known as B7-1) on their surface, which alters the shape of these cells and the integrity of the filter's slit-like openings, letting protein leak into the urine.

Normally this CD80 expression is brief and self-regulating, and researchers describe minimal change disease as involving a failure of that regulatory step, so the protein leak persists, even though the kidney tissue looks nearly normal under a standard microscope. Minimal change disease is described as the cause of idiopathic nephrotic syndrome in a large majority of young children, and it also occurs in adults, where it accounts for a smaller share of cases and can behave somewhat differently, sometimes needing a longer course of treatment.

Sudden swelling in the legs, ankles, face, or around the eyes, foamy urine from heavy protein loss, weight gain from fluid, and tiredness. It is the most common cause of nephrotic syndrome in children and also occurs in adults.
